The following Training Organizations’ curriculums are approved by the Kentucky Department for Public Health Office of Community Health Workers to provide Foundational (Tier 1) training for Community Health Workers.

Appalachian Kentucky Health Care Access Network

AKHCAN provides quality training to support Community Health Workers and other professionals as they address the needs of their communities. Our mission is to improve health care access and positive health outcomes for all those we serve.

Kentucky Homeplace

Kentucky Homeplace is recognized at the national, state, and local levels as a community health worker (CHW) model that works. Residents of rural Kentucky have unusually high levels of certain diseases, including cancer, heart disease, hypertension, asthma, and diabetes.

Lifestyle choices, environmental factors, inadequate health insurance, and general lack of understanding of the healthcare system are often cited as contributing factors. To help address these issues, Kentucky Homeplace began in 1994 as a demonstration project in 14 Appalachian Counties.

After more than two decades, Kentucky Homeplace CHWs have linked tens of thousands of rural Kentuckians with medical, social, and environmental services they otherwise might have gone without.

University of Louisville Trager Institute

Republic Bank Foundation Optimal Aging Clinic

The Trager Institute provides training for Community Health Workers and other healthcare professionals, with a focus on expanding access to behavioral health services across Kentucky. Our mission is to improve how people age and to change the way health care is delivered. Our trainings are trauma-informed and our FlourishCare model ensures that we train learners in interprofessional collaboration to provide holistic healthcare to meet all client needs and promote overall health.
